Essential Qualification: 1. The candidate should have passed B.Sc Nursing course 1GNM training course from any recognized institution. 2. The candidate should be able to read, write & speak in Bengali. 3. The candidate must be the permanent resident ofDakshin Dinajpur district 4. Age as on 0110112018 should not exceeding 40 years and not less than 21 years relaxation in age will be given for SC/ST OBC candidates as per extant rule and order in this regard).
Document to be submitted: 1. Self attested photo copy of photo identity card (Passport or EPIC or AADHAR card or PAN Card). 2. Self attested photo copy of document certifying permanent residential status of this district (Passport or EPIC or AADHAR card or PAN Card or Driving License or Ration card). 3. Self attested photocopy of Birth Certificate, Admit Cardl Certificate ofMP Examination or equivalent Examination affiliated by any Board recognized by Govt. of India or any State Govt. 4. Self attested photocopy of Caste Certificate if any. Candidates cJaming as EC candidate must submit self attested photocopy ofE.C. Certificate issued from the Competent Authority. 5. Self attested mark sheet and passed certificate of Secondary, Higher secondary and GNM training course or B.Sc Nursing course. 6. Self attested 02 (two) copies of recent self attested P.P size photograph. 7. Last date for submission of application: 17/04/2018 up to 5.00 p.m. at the office of the Chief Medical Officer of Health, Balurghat, Dakshin Dinajpur, Pin-733101 through Registry or Speed Post only. 8. Total posts: 04 [Scheduled Caste: -01, Unreserved: 01, Unreserved (EC): 01, Scheduled Tribe: 01] 9. An application fee in form of Demand Draft ofRs. 100.00 for General applicant and Rs. 50.00 for reserved categories to be submitted with the application. The Demand Draft should be in favor of District Health & Family Welfare Samiti, Dakshin Dinajpur payable at Balurghat. 10.
